Overview
Healthforce Healthcare LLC has a travel opportunity for an LPN with experience in long-term care. This 14-week assignment is scheduled for evenings (5x8) with a focus on geriatric and adult resident care. The role requires active licensure in New York and delivery of nursing care under supervision of a registered nurse. Orientation for full shifts must begin on a Tuesday or Thursday.

Responsibilities
- Provide direct and indirect skilled nursing care for residents under the supervision of an RN.
- Collect ongoing data on residents’ physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
- Contribute to development and execution of an age-appropriate, culturally relevant plan of care with residents and families, and implement nursing interventions.
- Reassess and revise the plan of care as resident outcomes are evaluated; initiate referrals as needed.
- Maintain professional knowledge and participate in continuing education and in-service activities; mentor and orient other staff as appropriate.
- Maintain safety and infection control practices; support a collaborative team environment with physicians and ancillary staff.
- Demonstrate competency in geriatric and adult care, ensuring care is aligned with resident goals and age-specific needs.

Requirements
- Active New York LPN license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience, preferably in long-term care
- Experience providing care to geriatric residents
- Ability to work evenings and participate in rotating shifts (D/E/N/W as applicable)
- Demonstrated ability to function under supervision of an RN and collaborate with the interdisciplinary team
- Must be able to begin full-shift orientation on a Tuesday or Thursday
