- Provide direct patient care to residents in accordance with the plan of care established by the supervising nurse and physicians.
- Administer prescribed medications and document treatments and responses in the resident’s chart.
- Monitor vital signs, observe for changes in condition, and report findings promptly to the appropriate team member.
- Assist residents with activities of daily living and provide support during admissions, transfers, and discharges.
- Collaborate with the interdisciplinary team, participate in shift handoffs, and contribute to care planning and quality improvement activities.
- Maintain safe and clean clinical environments, adhere to infection control protocols, and ensure compliance with facility policies and procedures.
Requirements
- Active Licensed Practical Nurse license (state to be verified)
- Experience in long-term care or a skilled nursing setting preferred
- Ability to work 12-hour shifts and meet the guaranteed 36 hours per week
- Strong documentation and communication skills
- Competence with basic nursing procedures and resident-centered care delivery
