Responsibilities
- Assist the supervising occupational therapist in evaluating and treating patients in the home health and hospice setting, focusing on functional abilities and activities of daily living.
- Implement prescribed therapy interventions and therapeutic modalities under supervision, adjusting activities as directed.
- Document evaluations, treatment goals, interventions, and progress notes in the patient record.
- Communicate patient status and progress with the interdisciplinary care team and participate in plan-of-care discussions.
- Educate patients and family members on home exercise programs, safety routines, and the rationale for treatments.
- Support discharge planning and transitions of care, collaborating with therapy and nursing teams to optimize outcomes.
Requirements
- License: Must hold a licensed Occupational Therapist license in Vermont and New Hampshire or an unencumbered compact license.
- Education: BS in Occupational Therapy or postgraduate Occupational Therapy Certification program.
- Certification: Must have passed the National Registration Exam.
- Experience: Home health setting experience is preferred.
- Ability to work in a home health/hospice setting and travel as required.
