Travel Physical Therapist Job
Physical Therapist travel assignment in Fairmont, West Virginia. Start date 04/06/2026 for 13 weeks. Weekly pay $2,051–$2,165 per week with 35 guaranteed hours. Shift is 08:00–16:30 on a 5x8 schedule in a long-term care/nursing home setting.

Responsibilities
- Perform comprehensive PT evaluations for residents and develop individualized treatment plans.
- Deliver therapeutic interventions including therapeutic exercise, gait and balance training, mobility training, ROM and strength work, and modality application as appropriate.
- Monitor progress, adjust plans as needed, and document treatment and outcomes in the resident’s chart.
- Collaborate with an interdisciplinary team to coordinate care, discharge planning, and education for residents and family members.
- Ensure safety, infection control, and privacy in all patient interactions; promote adherence to facility policies and documentation standards.
- Provide patient and family education on home exercise programs, fall prevention, and self-management strategies.
Requirements
- Active Physical Therapist license to practice in West Virginia (or eligible for WV licensure)
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
- CPR/BLS certification required
- Ability to work effectively in a long-term care setting and to meet credentialing and documentation requirements
- Relevant experience in physical therapy, with preference for long-term care or skilled nursing setting exposure
- Strong communication skills and ability to collaborate with an interdisciplinary team
