Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients per the nursing process and established standards.
- Supervise and direct activities of various levels of nursing staff; coordinate care with other disciplines.
- Administer medications and treatments as ordered; monitor patient status and respond to changes.
- Perform ongoing assessments, contribute to care planning, and evaluate patient outcomes.
- Educate patients and families on care plans, symptom management, and safety; ensure smooth coordination with interdisciplinary teams.
- Document care accurately and maintain compliance with policies and procedures in a home hospice setting.
Requirements
- Active RN license in Oregon (or eligibility to obtain) for professional practice.
- ICU or critical care nursing experience preferred; strong clinical assessment skills required for home hospice care.
- Current Basic Life Support (BLS) certification; ACLS may be preferred or required by setting.
- Ability to work autonomously in a home healthcare environment; comfortable coordinating with families and interdisciplinary teams.
- Willingness to travel within the assigned area to provide patient care during the scheduled shifts.
- Graduate of an accredited nursing program; demonstrated readiness for high-acuity care in a home setting.
