Overview
Physical Therapist – General, Travel Assignment in Bala Cynwyd, Pennsylvania, starting 09/16/2026 for 12 weeks. Travel - Temporary Assignment in a home health setting with 40 guaranteed hours per week. Shift is 8:00 AM–4:30 PM, Monday through Friday (5x8). Weekly pay ranges from $2,512 to $2,691. This position requires EPIC experience and OASIS proficiency and includes start-of-care and revisits visits as part of patient care.

Responsibilities
- Provide physical therapy services to home health patients per physician orders, including initial assessment and ongoing visits
- Develop and administer patient-specific treatment plans to improve mobility, strength, balance, and functional independence
- Conduct Start of Care (SOC) and revisits, document progress, and adjust interventions as needed
- Complete documentation in EPIC and perform OASIS assessments as required
- Collaborate with the interdisciplinary team and instruct caregivers on home exercise programs and safety

Requirements
- Active physical therapy license in Pennsylvania (or eligible state per licensure requirements)
- Doctor of Physical Therapy (DPT) degree
- Recent, relevant home health experience
- EPIC experience and OASIS proficiency required
- Ability to perform Start of Care and revisits in a home health setting
- Strong communication skills and ability to work independently in patients’ homes
