Responsibilities
- Provide direct patient care to adult residents in a long-term care setting, delivering evidence-based nursing support under supervision.
- Administer prescribed medications and therapies according to physician orders, facility policy, and patient safety protocols.
- Monitor residents’ physical and cognitive status, perform routine assessments, document findings, and promptly report changes to the nursing team.
- Assist with activities of daily living (ADLs) and restorative activities to promote independence, comfort, and quality of life.
- Maintain accurate and timely documentation for all care activities, including MARs, flowsheets, and progress notes.
- Collaborate with interdisciplinary team members (physicians, therapists, social workers, and dietary) to develop and adjust care plans; participate in rounds and care conferences.
- Uphold infection control, safety, and comfort measures; maintain a clean and safe environment for residents and staff.
Requirements
- Active Licensed Practical Nurse license in Rhode Island
- Graduation from an accredited practical nursing program
- Prior experience in long-term care or skilled nursing settings preferred
- Ability to work night shifts and a flexible schedule; comfortable providing care for adults with complex needs
- Strong clinical skills in assessment, documentation, medication administration, and patient safety
- Effective communication and teamwork to support a resident-centered care approach
