Travel Physical Therapist Job
CWHH-Physical Therapist (Physical Therapist) position in a Skilled Nursing Facility setting in Washington, NC. Start date 03/25/2026 for a 13-week travel assignment. Weekly pay range $2,464-$2,578 with guaranteed 40 hours per week on an 8-hour shift.

Responsibilities
- Perform clinical assessment to identify patient needs and appropriateness of physical therapy services.
- Document, plan, review and complete a Plan of Care (POC), progress notes, and other clinical records; revise the POC based on evaluation data; collaborate with the physician and interdisciplinary team.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic conditions based on medical referral and therapy assessments.
- Instruct patients and families in transfer techniques and safe use of equipment (walkers, wheelchairs, canes, braces, prosthetics); select and adapt rehabilitation equipment as needed.
- Provide patient/family education regarding therapeutic procedures, considering age, condition, culture and learning readiness.
- Oversee or supervise other team members as appropriate to patient care; apply infection control and universal precautions.
- Accept clinical assignments within education and competence; contribute to coordinated care within the care team.
Requirements
- Active Physical Therapist license in North Carolina
- Proof of current CPR certification
- Bachelor of Science in Physical Therapy from an APTA-approved program
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting; recent home health experience preferred
- Current driver's license with reliable transportation and valid auto insurance
- Ability to work in a travel assignment within a Skilled Nursing Facility setting; capability for heavy lifting and prolonged standing
