Responsibilities
- Evaluate and treat patients using medically prescribed physical therapy programs to restore function and minimize disability
- Conduct initial admissions/evaluation visits and develop individualized PT plans of care
- Provide ongoing skilled interventions and therapeutic activities to meet patient goals
- Supervise Physical Therapy Assistants as assigned and coordinate care within the clinical team
- Document observations, clinical findings, problems, goals, interventions, and discharge plans; notify the Clinical Team Manager of OASIS scores within established timelines
- Perform duties aligned with the goals and mission of the home health and hospice program while maintaining quality and compliance
Requirements
- One year of Home Health experience
- Ability to perform initial admissions/evaluations and implement medically prescribed PT programs
- Ability to supervise Physical Therapy Assistants as assigned
- Strong documentation skills including observations, clinical findings, goals, and discharge plans
