Responsibilities
- Provide physical therapy evaluations and therapeutic interventions for adult patients within the assignment setting
- Develop, implement, and monitor individualized plans of care in collaboration with the care team
- Reassess patient progress regularly and adjust treatment plans to meet rehabilitation goals
- Document clinical findings, progress notes, and outcomes in the patient record timely and accurately
- Educate patients and families on treatment plans, safety, and discharge goals; participate in discharge planning and care coordination
- Ensure compliance with applicable regulations, ethical standards, and facility protocols
Requirements
- Education: Doctor of Physical Therapy (DPT) required if degree obtained after 2016
- Certifications: Basic Life Support (BLS) required
- Experience: Prior home health experience (1 year would suffice) or willingness to train
- Age: Must be 21 years or older
- Additional: Eligibility to practice as a physical therapist in the assigned state; ability to work in the specified setting
