Responsibilities
- Deliver skilled nursing care in patients’ homes, including assessment, vital signs monitoring, wound care, medication administration, and IV therapy within scope
- Complete OASIS data collection and documentation accurately per agency protocols
- Develop and update the Plan of Care in collaboration with the interdisciplinary team; communicate with patients, families, and caregivers
- Manage a high-volume visit schedule (approximately 5–6 patients per day) with thorough, timely documentation
- Ensure patient safety and quality of care; adhere to standard precautions, infection control, and agency policies; escalate concerns as needed
Requirements
- Active Registered Nurse license in New Jersey (NJ) and must be a local candidate
- Minimum 2 years of home health nursing experience
- Proficient in OASIS data collection and documentation
- Availability for 1 weekend out of every 4
- Ability to perform duties in a home health setting and travel to patient homes; strong communication and documentation skills
