Responsibilities
- Provide direct nursing care to residents in a long-term care setting, addressing daily needs and changing conditions
- Administer prescribed medications and treatments in accordance with care plans and safety protocols
- Monitor residents’ vital signs, assess for changes, and report findings to the supervising RN
- Assist with activities of daily living, mobility, hygiene, nutrition, and comfort measures
- Document care, interventions, and resident responses accurately in the electronic health record
- Collaborate with the interdisciplinary team to implement individualized care plans and ensure continuity of care
Requirements
- Valid and active Licensed Practical Nurse (LPN) license in New Hampshire
- Minimum 1 year of clinical nursing experience in long-term care or a similar setting preferred
- Current CPR/BLS certification or ability to obtain prior to start
- Ability to work night shifts (12-hour shifts) and participate in a temporary travel assignment
- Strong documentation, communication, and teamwork skills
- Able to pass pre-employment screenings and meet required immunization or health-related criteria
