Responsibilities
- Conduct psychosocial assessments for patients and families, document findings, and contribute to care planning
- Develop and implement effective discharge plans, coordinating with medical teams and ancillary services
- Provide counseling and support to patients and families facing illness, hospitalization, or transitions in care
- Identify and arrange appropriate community resources, financial assistance, and advance directive discussions as appropriate
- Collaborate with interdisciplinary teams to ensure smooth continuity of care and timely follow-up after discharge
- Document all activities in the electronic health record per policy and maintain compliance with applicable regulations
- May support other clinics remotely, providing consultation and care coordination as needed
Requirements
- Education: Bachelor’s degree in Social Work or related field; Master’s degree in Social Work required
- Experience: 3+ years of clinical/social work experience strongly preferred; hospital clinical work experience preferred
- Certifications/Licenses: LCSW or CSWA preferred
- Skills: Strong interpersonal and communication skills; skilled in assessment, counseling, resource coordination, and documentation
- Work setting: Ability to work onsite in a medical/clinical environment; capable of supporting remote clinics as needed
