Responsibilities
- Provide direct patient care to residents within the long-term care setting, including assessments, vital signs monitoring, and assistance with daily activities within your scope of practice.
- Monitor residents for condition changes, promptly report to the supervising nurse, and collaborate with the interdisciplinary team to adjust care plans as needed.
- Administer prescribed medications and treatments accurately, safely, and in accordance with policy, physician orders, and regulatory guidelines.
- Support resident mobility and rehabilitation goals, assist with activities of daily living, and promote resident dignity, comfort, and safety.
- Maintain accurate documentation in the electronic health record; complete clear shift handoffs to ensure continuity of care.
- Adhere to infection control protocols, safety standards, and quality assurance procedures; participate in care planning, rounds, and performance improvement activities.

Requirements
- Active Licensed Practical Nurse license in New Hampshire (NH)
- Completion of an approved practical nursing program (Diploma or Associate Degree)
- CPR/BLS certification or ability to obtain prior to start
- Minimum 1 year of LPN experience in a long-term care setting preferred
- Ability to work the specified evening shift (14:30–23:00) and be local to Lancaster, NH
- Strong communication, documentation, and teamwork skills; ability to function effectively in a dynamic, regulated care environment
