Responsibilities
- Perform comprehensive physical therapy evaluations for residents in a skilled nursing setting.
- Develop and implement individualized treatment plans to improve mobility, strength, and function.
- Deliver therapeutic exercises, gait training, balance activities, and modalities per plan.
- Monitor progress, adjust treatment plans as needed, and document outcomes accurately.
- Communicate and collaborate with an interdisciplinary care team to optimize resident outcomes and safety.
- Ensure compliance with regulatory requirements, documentation standards, and facility policies.
Requirements
- Active West Virginia physical therapy license
- Ability to work effectively in a skilled nursing facility setting
- Willingness to commit to a 13-week assignment in Hilltop, WV
- Strong communication and teamwork skills
- Ability to manage a schedule with guaranteed hours and the listed shift pattern
