Responsibilities
- Conduct initial PT assessments for residents and develop individualized treatment plans aimed at mobility, strength, balance, gait, and functional independence.
- Deliver evidence-based, goal-directed interventions and monitor patient progress, adjusting plans as needed.
- Reassess patients regularly, document progress in the electronic health record, and communicate changes to the care team.
- Collaborate with nursing, therapy assistants, physicians, and other staff to coordinate care and discharge planning.
- Educate residents and families on exercises, safety strategies, and home programs to support continued improvement after discharge.
- Maintain safety, infection control, and documentation standards in accordance with facility policies and regulatory requirements.
Requirements
- Active Physical Therapist license in New Jersey or eligibility to obtain licensure in New Jersey.
- Ability to work a 13-week travel assignment and commit to 35 hours per week.
- Comfortable providing PT services in a skilled nursing facility setting with adult residents.
- Strong written and verbal communication skills; ability to work collaboratively within an interdisciplinary team.
- Availability to work the stated shift (08:00–16:30) and adapt to the assignment schedule.
