Responsibilities
- Conduct initial occupational therapy evaluations for residents in the skilled nursing facility, assessing functional mobility, activities of daily living, balance, cognition, and safety with mobility.
- Develop and implement individualized treatment plans; provide direct therapy and adjust plans based on progress; document progress and outcomes.
- Deliver therapy aimed at improving independence, endurance, strength, coordination, and cognitive skills; educate residents and families on home exercise programs and safety.
- Collaborate with the interdisciplinary team; participate in rounds; support discharge planning and transitions to home or other care settings.
- Supervise therapy assistants or support staff as assigned; ensure adherence to scope of practice and facility policies.
- Maintain compliant, timely documentation in the electronic health record and ensure HIPAA compliance.
Requirements
- Active OT license or eligibility to practice in the United States; California licensure may be required or eligibility to obtain licensure in the state.
- Ability to work in a skilled nursing facility setting with an interdisciplinary team.
- Strong clinical assessment, treatment planning, and documentation skills; proficient with electronic medical records.
- Excellent communication with residents, families, and the care team; dependable and able to meet guaranteed hours.
- Availability for a 13-week travel assignment and the specified 5x8 schedule starting 12/01/2026.
- Preferred experience in geriatric rehabilitation or SNF settings.
