Responsibilities
- Provide direct, nurse-led care to residents in a long-term care setting, including monitoring conditions and implementing care plans under supervision of an RN.
- Administer medications and treatments per physician orders, document administration accurately, and monitor for adverse effects.
- Perform ongoing assessments, measure vitals, monitor hydration and nutrition status, skin integrity, and mobility; escalate changes promptly to the supervising clinician.
- Assist with activities of daily living, wound care, hygiene support, specimen collection, and safe transfers using appropriate mobility aids.
- Collaborate with the interdisciplinary team—nursing, therapy, social work, and dietary staff—to update and execute individualized care plans; participate in rounds as required.
- Maintain precise documentation in the electronic medical record, ensure compliance with regulatory standards and facility policies, and participate in quality improvement activities.
Requirements
- Active Vermont Licensed Practical Nurse license (or eligibility to practice in Vermont)
- Current CPR/BLS certification
- Graduation from an accredited practical nursing program
- Ability to work evenings and weekends as required
- Successful completion of background check and pre-employment drug screen
- Prior experience in a long-term care or skilled nursing setting is preferred
