Responsibilities
- Administer medications and treatments within the scope of practice and facility policy
- Monitor residents’ condition, document vital signs, intake/output, and report changes to the supervising RN
- Assist with activities of daily living and provide basic nursing care for residents
- Collaborate with the Registered Nurse, interdisciplinary team, and care plans to ensure safe, individualized care
- Maintain accurate, timely documentation and uphold infection prevention and safety standards
- Respond to resident needs and call lights promptly, ensuring resident safety during night shifts
Requirements
- Active Licensed Practical Nurse license in New Hampshire or eligibility to practice in NH
- Current Basic Life Support (BLS) certification or ability to obtain
- Graduation from an approved practical nursing program
- Minimum of 1 year of licensed practical nursing experience in long-term care or skilled nursing setting (preferred)
- Ability to work night shifts (7:00 PM–7:00 AM) and participate in a travel assignment
- Access to appropriate means to comply with background check and drug screening as part of the employment process
