Responsibilities
- Administer standard physical therapy treatments for disabilities, injuries, and diseases to relieve pain, restore motor function, and maintain optimal performance.
- Complete patient evaluations within 24 hours of physician orders and discharge forms within 24 hours of discharge.
- Document patient assessment and ongoing care concisely, preparing admission forms, plan of treatment, progress notes, and other forms per state regulations and policy.
- Plan health care services based on physician orders and patient needs; coordinate with the health care team and resources in the community; promote patient and family independence for discharge.
- Supervise treatment administration by PT Assistants and aides; instruct home care personnel on lifting techniques and body mechanics; ensure proper use of equipment.
- Attend case conferences and staff meetings; contribute to performance improvement activities; maintain compliance with licensure, certifications, and education requirements.
- Orient and supervise new PT staff in home health processes when applicable; promptly communicate changes in patient condition or orders to the team.
- Maintain ongoing collaboration with the rehabilitation director and other team members to coordinate referrals and patient care.

Requirements
- Active licensure/credentialing requirements to practice physical therapy as defined by local administration and regulatory bodies.
- Education, certifications, and ongoing compliance aligned with state and agency standards.
- Ability to work in an outpatient rehabilitation setting and contribute to team-based care.
- Strong documentation, assessment, and communication skills to support timely evaluations and discharge planning.
- Experience in coordinating care and supervising PT Assistants and aides as part of a multidisciplinary team.
- Professionalism and commitment to patient-centered care in a fast-paced outpatient environment.
