Responsibilities
- Provide occupational therapy evaluation and timely treatment for residents with functional impairments related to acute care needs.
- Develop and implement individualized care plans with measurable short- and long-term goals.
- Perform standardized assessments, monitor progress, and revise interventions as needed.
- Collaborate with an interdisciplinary team to support discharge planning, transitional care, and ongoing rehabilitation goals.
- Document patient progress and care activities in the chart accordance with regulatory and facility policies; ensure accuracy and compliance.
- Maintain safety, infection control, and hand hygiene practices; adapt activities to residents’ tolerance and safety levels.
Requirements
- Active occupational therapy license in California or eligibility to practice in CA.
- Ability to work in a skilled nursing facility setting and contribute to an interprofessional team.
- Willingness to commit to a travel assignment with guaranteed hours (18 weeks).
- Strong documentation, communication, and interpersonal skills.
- Demonstrated flexibility to adapt to rehab needs within an acute care-oriented SNF environment.
