Responsibilities
- Perform comprehensive occupational therapy evaluations for residents to determine functional abilities and goals
- Develop and implement individualized treatment plans aimed at improving activities of daily living, mobility, and safety
- Deliver therapy interventions addressing coordination, strength, cognition, fine motor skills, and adaptive equipment use
- Monitor progress, document outcomes, and adjust plans in collaboration with the care team
- Educate residents and families on home exercise programs, safety strategies, and caregiver instruction
- Participate in discharge planning, care coordination, and interdisciplinary team meetings to optimize patient outcomes
Requirements
- Ability to provide occupational therapy services in a skilled nursing facility setting
- Strong documentation and communication skills
- Demonstrated collaboration with an interdisciplinary healthcare team
- Willingness to participate in a travel assignment
- Availability to work guaranteed hours as specified (35 per week)
