Responsibilities
- Provide direct basic nursing care to residents within the scope of an LPN, under supervision
- Administer prescribed medications and document administration and observations in MatrixCare
- Monitor resident status, vital signs, and respond to changes; communicate changes to the supervising RN
- Assist residents with activities of daily living and comfort measures, supporting dignity and independence
- Collaborate with the interdisciplinary care team to implement care plans, ensure infection control, and maintain safety
- Complete required documentation, maintain accurate MARs, and participate in shift handoffs
- Schedule adherence as outlined by the assignment, including PT 2 days per week and every other weekend
Requirements
- Active Pennsylvania Licensed Practical Nurse (LPN) license
- Minimum of 1 year LTC experience
- Experience with MatrixCare documentation system
- Ability to work the 3:00 pm – 11:00 pm shift
- Eligible for a travel/temporary assignment and able to perform duties in a long-term care setting
