Responsibilities
- Provide direct nursing care to residents in a long-term care/assisted living setting, aligning with care plans
- Monitor vitals, administer medications and treatments per orders, and document changes accurately
- Assist residents with activities of daily living and mobility, supporting safety and dignity
- Collaborate with an interprofessional team to implement care plans and respond to resident needs
- Maintain thorough and compliant documentation, including incident reports and care notes
- Ensure infection control practices, patient safety, and adherence to regulatory requirements
- Respond promptly to changes in resident condition, escalating concerns to the charge nurse as needed
Requirements
- Active LPN license in Minnesota
- Minimum regional travel experience (travel experience preferred)
- Ability to work Evening and Night shifts with guaranteed 40 hours per week
- Must report to the assigned facility within 72 hours of offer
- Completion of an approved practical nursing program
- Ability to work in a long-term care/assisted living environment and collaborate with a multidisciplinary team
