Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
- Collect relevant health data, analyze assessment information, determine diagnoses and care needs, and develop a plan with prescribed interventions.
- Implement the plan, coordinate care delivery, and promote a safe, healing environment; supervise and direct activities of various levels of nursing staff; collaborate with other disciplines.
- Evaluate progress toward outcomes; identify anticipated outcomes and participate in discharge planning to ensure continuity of care.
- Provide education and information to patients and families; document care and communications; delegate appropriately and coordinate duties of the healthcare team.
Requirements
- Active Registered Nurse license in the state of practice (California)
- Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification required
- EPIC experience required
- Minimum two years of recent nursing experience in the field
- BSN preferred; willingness to work in a home healthcare - hospice setting
- Ability to work night shift and meet assignment needs
