Responsibilities
- Provide diagnostic, therapeutic and consultative services to patients with communication, cognitive and swallowing disorders.
- Develop and implement individualized treatment plans; conduct assessments and monitor patient progress.
- Document evaluations, treatment plans, and progress notes in accordance with professional standards and regulatory requirements.
- Collaborate with an interdisciplinary care team to coordinate patient care, goals, and discharge planning.
- Adhere to applicable state and federal license, certification and accrediting guidelines.
- Educate patients and families on home-based therapy strategies and carryover activities to support functional communication and swallowing goals.

Requirements
- Master’s degree in Speech-Language Pathology from an accredited program
- California state licensure as a Speech-Language Pathologist (or eligible to obtain)
- Ability to provide diagnostic, therapeutic and consultative services for communication, cognitive and swallowing disorders
- Knowledge of state and federal licensure, certification and accrediting requirements
- Ability to work in a home healthcare - hospice setting and participate as part of an interdisciplinary team
