Responsibilities
- Administer prescribed medications and treatments to residents, following physician orders and facility policy
- Monitor residents’ conditions, document observations, and report changes to the supervising RN promptly
- Provide direct nursing care and assistance with activities of daily living (ADLs) as needed
- Perform routine nursing procedures within scope of practice and maintain accurate, timely charting in the resident’s record
- Support care plans, collaborate with the interprofessional team, and participate in shift handoffs
- Ensure infection control practices, safety protocols, and resident comfort while maintaining professional, compassionate communication
Requirements
- Current and unrestricted Licensed Practical Nurse (LPN) license in New Hampshire
- Graduation from an approved LPN program (diploma or certificate)
- Basic Life Support (BLS/CPR) certification
- Ability to perform and sustain three 12-hour day shifts weekly and work the specified schedule
- Strong communication skills and ability to document care accurately
- Experience in or willingness to work in a long-term care setting in Lebanon, NH
