Responsibilities
- Provide direct nursing care to residents per established care plans, under supervision
- Administer prescribed medications and document administration accurately
- Monitor vital signs, assess resident status, and report changes promptly to the supervising clinician
- Assist with activities of daily living (ADLs) and mobility, ensuring safety and dignity
- Perform basic nursing procedures (wound care, injections, dressing changes) within scope of practice
- Maintain accurate charting, collaborate with interdisciplinary team, and support infection control and safety protocols
Requirements
- Active Licensed Practical Nurse license in Vermont (or eligibility for licensure in Vermont)
- Current CPR certification
- Experience in long-term care or skilled nursing settings preferred
- Ability to work evening and night shifts
- Effective communication and teamwork skills
