Responsibilities
- Deliver direct patient care to residents during night shifts, including monitoring status and comfort.
- Administer medications and treatments according to physician orders; document accurately in the Point-Click-Care system.
- Monitor vital signs, assess for changes, and report findings promptly to the supervising nurse.
- Assist residents with activities of daily living, mobility, hygiene, and safety, while maintaining dignity and independence.
- Collaborate with the interdisciplinary team to implement individualized care plans and ensure patient safety and quality of care.
- Complete comprehensive shift handoffs and maintain clear, timely documentation in the electronic health record.
Requirements
- Point-Click-Care system experience REQUIRED
- Availability for night shifts (12-hour) and 3x12 schedule
- Availability to work every other weekend
- Travel assignment eligibility and WV work authorization
- Demonstrated ability to work in a LTC setting with strong communication and teamwork skills
