Responsibilities
- Deliver direct patient care to hospice patients in the home setting with emergency-level acuity as needed.
- Monitor patient condition, respond to changes, and administer medications per physician orders.
- Manage symptoms and provide comfort-focused interventions; ensure safety and adherence to care plans.
- Document care accurately in charting; maintain current, compliant records and timely communications with the care team.
- Collaborate with interdisciplinary team members and coordinate with supervisors and physicians as thresholds change.
- Educate patients and families on care plans, medication safety, and at-home safety practices.
Requirements
- Ability to work night shifts (3x12 hours) in a home-based hospice environment.
- Willingness to participate in a travel assignment and adapt to varied patient locations.
- Comfortable working in a home healthcare setting with potentially high-acuity needs.
