Responsibilities
- Conduct comprehensive assessments for residents with speech, language, cognitive-communication, voice, fluency and swallowing concerns.
- Develop and implement individualized treatment plans; document progress and outcomes.
- Provide direct therapy and consult with residents, families, and interdisciplinary team members (nursing, rehabilitation therapies, social work).
- Monitor progress, adjust treatment plans as needed, and participate in discharge planning and care transitions.
- Ensure accurate documentation and regulatory compliance; maintain patient privacy and safety standards.
- Participate in care conferences and interdisciplinary team meetings to optimize outcomes for residents.
Requirements
- Active Speech-Language Pathologist license in California
- Ability to work in a skilled nursing facility environment
- Strong communication, collaboration, and documentation skills
- Eligible to work in the United States for a travel assignment
- Demonstrated ability to use standardized assessment tools and EHR/documentation systems
