Responsibilities
- Provide emergency and palliative nursing care to home hospice patients, identifying and responding to acute changes in condition.
- Administer medications per orders, including IV therapies and PRN medications, while monitoring responses and safety.
- Perform ongoing patient assessments, monitor vital signs, document findings, and escalate concerns to the medical team as needed.
- Collaborate with the interdisciplinary hospice team to develop and implement individualized care plans and symptom management.
- Educate patients and families on care plans, medications, safety at home, and expected disease trajectory.
- Maintain accurate documentation in the patient record and ensure compliance with regulatory and agency standards.
Requirements
- Active RN license in California (state of practice) or eligibility as required to work in the location.
- CPR/BLS certification.
- Ability to work 12-hour night shifts and adapt to a home hospice environment.
- Strong clinical assessment skills, clinical judgment, and communication abilities.
- Willingness to travel to patient homes within the assignment area and collaborate with the hospice team.
- Comfort with managing urgent and emergent situations in a home setting while providing compassionate patient-centered care.
