Travel Registered Nurse Case Manager Job
Registered Nurse – Case Manager position in Everett, Washington. Travel assignment at a Level II trauma center hospital. Start date 06/29/2026 for 13 weeks. Shift is 4x8 days. Weekly pay range is $2,173 to $2,274 per week. Assignment type: Travel – Temporary. This role focuses on acute care discharge planning and care coordination across hospital units with cross-campus coverage as needed.

Responsibilities
- Perform acute care discharge planning and care coordination within an acute care hospital system.
- Conduct needs assessments, develop plans of care, and coordinate discharge to the next level of care; arrange DME as needed; ensure alignment with CMS processes.
- Collaborate with Social Services and an interdisciplinary team to support diverse patient populations across units; document in EPIC and maintain thorough, compliant records.
- Manage a caseload of approximately 25–30 patients; provide coverage across like units and two campuses (Colby and Pacific campuses) with appropriate floating.
- Support efficient transitions of care, remove barriers to discharge, and participate in orientation and pre-start modules as required.
Requirements
- Licensure: Active Registered Nurse license in Washington; or Master of Social Work (MSW) per posting.
- Certifications: BLS required; RN licensure as listed; additional certifications per posting as applicable.
- Experience: Minimum 1 year in acute care with discharge planning / care coordination; diverse case management experience preferred (Med Surg, LLOS, ED).
- Education: RN degree (ADN/BSN) or MSW.
- Skills: Proficiency with EPIC charting; familiarity with CMS-related processes; ability to float across units and campuses as needed.
