Medical Social Worker (MSW) – Everett, WA | Start 06/29/2026 | 13 weeks | $1,677-$1,815 per week | 72 hours per 2 weeks guaranteed | Shift: 5x8 Days | Travel - Temporary Assignment
This travel MSW role supports adults and geriatrics in a hospital setting. The assignment begins in late June and runs for approximately 13 weeks, with a 5x8 day shift and cross-unit coverage as needed. EPIC charting, discharge planning, and care coordination are core aspects of the position.

Responsibilities
- Conduct psychosocial assessments for adult and geriatric patients; identify barriers to discharge and plan accordingly
- Develop and manage discharge planning and care coordination in collaboration with the interdisciplinary team
- Facilitate referrals to home health, rehabilitation, long-term care, and other community resources; coordinate DME and transportation as needed
- Document care plans, progress, and outcomes in the electronic medical record (EPIC) and ensure compliance with organizational policies
- Collaborate with PT, Interpretation Services, Social Services, Pharmacy, and other teams; float across hospital units as required
- Provide patient and family education on resources, advance directives, coping strategies, and post-discharge planning
Requirements
- Active Washington state license to practice social work
- BLS certification
- EPIC charting experience
- Minimum 1 year of acute care experience with discharge planning/care coordination
- Ability to float across hospital units and adapt to multiple care settings
- Strong communication and interdisciplinary collaboration skills
