Licensed Practical Nurse (LPN) – Long-Term Care with memory care in Gunnison, Colorado. Responsibilities
- Provide direct nursing care to memory-care residents, including medication administration, treatments, and monitoring of vital signs.
- Assess residents, document changes, and communicate with the nursing leadership and medical team.
- Administer medications and treatments per plan of care; perform IV therapy if certified.
- Supervise and coordinate day-to-day nursing activities of care staff during the shift under the direction of the Director of Nursing.
- Collaborate with the interdisciplinary team to support rehabilitation and restorative nursing goals while maintaining safety, privacy, and regulatory compliance.
- Ensure adherence to applicable Federal, State, and local standards and participate in care planning as needed.
Requirements
- Active Colorado LPN license
- Current BLS certification; IV certification preferred
- Minimum 3 years’ experience in a hospital, skilled nursing facility, or comparable setting; training in rehabilitative and restorative nursing
- Graduation from an accredited LPN program (diploma or certificate)
- Ability to work 12-hour night shifts in a memory-care setting
- Colorado CAPS check required; reliable transportation due to rural Gunnison location
