- Document treatments, progress, and outcomes in the medical record per facility policy and regulatory requirements.
- Collaborate with the interdisciplinary team (nursing, occupational therapy, speech therapy, physicians) to coordinate care and discharge planning.
- Ensure patient safety, proper documentation, and infection control during therapy sessions.
Requirements
- Current, valid Physical Therapist license to practice in New Jersey or eligibility for NJ licensure.
- Ability to work in a skilled nursing facility environment and participate in a 12-week travel assignment.
- Strong documentation and communication skills; comfortable collaborating with an interdisciplinary team.
- Availability to start on or before 10/18/2026 and commit to the 12-week assignment in Cedar Grove, NJ.
