Responsibilities
- Provide routine nursing care to residents in a long-term care/assisted living setting under supervision of a registered nurse.
- Administer medications and treatments per physician orders; monitor for adverse effects and document responses.
- Assess residents, record vitals, and report any changes to the care team promptly.
- Assist residents with activities of daily living and mobility, while promoting independence and safety.
- Update and contribute to individual care plans; communicate changes to the interdisciplinary team to ensure continuity of care.
- Maintain accurate documentation and adhere to infection control and safety protocols; ensure a clean, safe care environment.
Requirements
- Current, active Licensed Practical Nurse license in Vermont.
- Graduation from an approved practical nursing program.
- Experience in long-term care or similar setting preferred.
- Ability to work in a long-term care/assisted living environment and to meet the assignment’s 40-hour weekly requirement.
- Strong communication and documentation skills; able to collaborate with a multidisciplinary team.
