Responsibilities
- Deliver direct, patient-centered care to long-term care residents under supervision of a registered nurse
- Administer prescribed medications and treatments as delegated and per policy
- Monitor resident conditions, document vital signs and responses, and report changes to the supervising nurse
- Assist residents with activities of daily living and mobility to support independence
- Maintain a safe, clean, and therapeutic environment with adherence to infection control and safety protocols
- Communicate effectively with the care team, participating in care planning and daily rounds

Requirements
- Active Licensed Practical Nurse (LPN) license
- Minimum 1 year of clinical experience in a long-term care or skilled nursing setting preferred
- CPR/BLS certification (or equivalent)
- Graduation from an approved LPN program
- Ability to work the day shift and participate in a travel assignment
- Strong communication and documentation skills
