Responsibilities
- Perform initial and ongoing physical therapy evaluations for adult residents.
- Develop and implement individualized treatment plans to address mobility, strength, balance, gait, and functional independence.
- Deliver evidence-based therapeutic interventions and document progress in the resident’s chart.
- Collaborate with the interdisciplinary care team to adjust plans of care and optimize outcomes.
- Educate residents and families on prescribed home exercise programs and safety strategies.
- Ensure adherence to facility policies, regulatory requirements, and patient safety standards.
Requirements
- Active physical therapy license in Texas or eligibility to obtain licensure.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- 1+ year of PT experience in geriatric or long-term care rehabilitation settings (preferred).
- Ability to work on-site in a nursing center setting and communicate effectively with residents, families, and staff.
- Current CPR/BLS certification (or willingness to obtain).
