Responsibilities
- Provide in-home nursing care to patients within the service area, delivering safe and evidence-based clinical interventions.
- Conduct comprehensive nursing assessments during visits; monitor vitals; administer prescribed medications; perform wound care and symptom management; educate patients and family caregivers.
- Document all care provided and patient status in the electronic health record; communicate changes to the supervising clinician and care team.
- Coordinate with the interdisciplinary team to implement care plans and participate in patient enrollment activities for the home-based program.
- Ensure safety, infection control, and quality standards during every home visit.

Requirements
- Active RN license in Pennsylvania (or eligible to practice in the state)
- Current BLS certification
- Ability to travel to patient homes and work in a home healthcare setting
- Flexibility to work rotating 12-hour shifts and in-home visits; overnight on-call shifts (approximately 1–2 per week) as needed
- Strong communication and documentation skills; proficiency with electronic health records
