Responsibilities
- Provide comprehensive physical therapy evaluations, develop individualized treatment plans, and implement evidence-based interventions for residents in a skilled nursing setting.
- Monitor progress, perform re-evaluations as needed, and adjust treatment plans to maximize functional outcomes and safety.
- Document evaluation findings, treatment plans, progress notes, and discharge summaries in the facility’s electronic medical record system.
- Collaborate with an interdisciplinary team (nursing, therapy aides, and social work) to coordinate care and discharge planning.
- Educate residents and families on treatment goals, home exercise programs, and safety strategies to support independence.
- Maintain professional and regulatory standards, including adherence to infection control and documentation requirements.
Requirements
- Active, unrestricted Physical Therapist license in California
- Graduation from an accredited physical therapy program (DPT preferred)
- Experience in geriatric or skilled nursing facility settings is preferred
- Ability to work effectively in a fast-paced, team-based care environment
- Availability for a 13-week travel assignment starting 04/13/2026
- Strong communication skills and commitment to resident-centered care
