Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings in the clinical record.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Gather data on physical, psychological, social, spiritual, and cultural factors; incorporate these data into the POC and maintain up-to-date patient records documenting visits, problems, plans, actions, and goals.
- Educate patients and families regarding care, medications, home safety, and other relevant topics; adapt teaching to age, condition, culture, and learning readiness; promote self-care practices.
- Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure continuity and quality of service; manage pain assessment and pain management education.
- Coordinate referrals to appropriate agencies; monitor cases for third-party payer requirements; participate in on-call responsibilities as assigned; communicate effectively with patients, families, team members, and physicians; attend interdisciplinary team meetings as needed.
Requirements
- Licensure: Active, current RN license in the appropriate state
- Education: ADN or Nursing Diploma required; BSN preferred
- Experience: 1-2 years RN clinical care experience; 2+ years home health experience required
- Certifications/Skills: CPR certification required; CHPN certification preferred
- Additional: Current CPR certification; Homecare Homebase software experience required; OASIS experience required; current driver’s license with reliable transportation and auto insurance; ability to travel locally
- Travel readiness: This travel assignment requires local travel to patient homes and flexible scheduling as needed
