Responsibilities
- Provide direct nursing care to residents under supervision of the nursing leadership
- Administer prescribed medications and treatments per the plan of care
- Monitor vital signs, assess changes in condition, and document responses to interventions
- Assist residents with activities of daily living and mobility as needed
- Perform wound care and dressing changes as assigned and maintain aseptic technique
- Communicate with RNs, physicians, and interdisciplinary team members; report changes promptly
- Maintain accurate, timely documentation in the medical record and uphold patient confidentiality
Requirements
- Active LPN license in good standing (state-specific as required by the assignment)
- Current Basic Life Support (BLS) certification
- Diploma from an accredited practical nursing program
- Prior LPN experience in long-term care or skilled nursing preferred
- Ability to work effectively in a long-term care setting with a rotating schedule
