Responsibilities

- Collaborate with the interdisciplinary team to coordinate goals, progress, and discharge planning
- Educate residents and families on therapy plans, safety, and home exercise programs
- Ensure compliance with regulatory and facility policies, including documentation standards
Requirements
- Ability to work effectively in a skilled nursing facility setting
- Strong clinical assessment, documentation, and time-management skills
- Excellent communication with patients, families, and care teams
