Travel Licensed Practical Nurse LPN Job
Licensed Practical Nurse (LPN) for a travel temporary assignment in a long-term care (LTC) setting in Rochester, New Hampshire. Start date 09/07/2026 for a 4-week contract with guaranteed 40 hours per week. Evening shift (3:00 PM–11:30 PM) on a 5x8 schedule. This is a travel opportunity managed by Healthforce Healthcare LLC.

Responsibilities
- Administer meds and treatments as prescribed, following the care plan and facility policies
- Monitor patient status, perform routine assessments, document vital signs, and report changes to the supervising RN
- Provide basic nursing care and support with activities of daily living (ADLs)
- Maintain accurate and timely documentation in patient records and care plans
- Collaborate with the interdisciplinary team to ensure safe, quality resident care
- Uphold infection control standards and resident safety; respond to call bells and emergencies promptly
Requirements
- Active LPN license in good standing (state of NH preferred)
- Current Basic Life Support (BLS) certification
- Prior experience in a long-term care or skilled nursing setting preferred
- High school diploma or equivalent; completion of an accredited practical nursing program
- Ability to work evenings and perform required nursing duties in a LTC environment
- Strong communication and documentation skills; ability to collaborate with an interdisciplinary team
