Responsibilities
- Evaluate patients’ physical therapy needs and develop individualized treatment plans
- Implement therapeutic interventions to improve mobility, strength, balance, and functional independence
- Conduct gait training, ROM exercises, and activity progression tailored to each patient
- Regularly reassess progress and modify treatment plans accordingly
- Document evaluations, treatments, and outcomes in the patient record; communicate findings with the interdisciplinary team
- Educate patients and families on home exercise programs, safety, and progress toward goals
Requirements
- This is a travel assignment requiring on-site work in Montana
- Ability to perform PT evaluations and deliver evidence-based interventions in a skilled nursing setting
- Willingness to collaborate with an interdisciplinary care team
- Commitment to a 13-week assignment with guaranteed weekly hours
- Proficiency with standard documentation and outcomes tracking in electronic health records
