Responsibilities
- Perform PT evaluations for residents and contribute to the development of individualized treatment plans
- Implement rehabilitation interventions including therapeutic exercise, gait training, balance work, and functional activities
- Continuously monitor progress, adjust plans as needed, and document outcomes and discharge planning
- Collaborate with nursing staff and other rehabilitation professionals to optimize functional independence and safety
- Maintain accurate, timely documentation in accordance with policy and regulatory requirements
- Educate residents and families on exercises, safety, and strategies to support outcomes and ongoing care
Requirements
- Active Physical Therapist license in the state of practice or eligibility for licensure by endorsement
- Doctor of Physical Therapy (DPT) degree or equivalent clinical education preferred
- Prior experience in skilled nursing or long-term care settings preferred
- Strong communication and teamwork skills; ability to work within an interdisciplinary team
- Ability to manage a 13-week travel assignment and accommodate a flexible schedule as needed
- Commitment to patient safety and adherence to facility policies and state regulations
