Responsibilities
- Administer prescribed medications and other routine nursing treatments per the plan of care.
- Monitor residents’ condition, perform daily assessments, document findings, and communicate changes to the supervising clinician.
- Assist with activities of daily living and basic care needs, ensuring patient safety and dignity.
- Collaborate with registered nurses and care teams to implement and adjust care plans.
- Respond to call lights promptly, manage pain and comfort measures, and support fall prevention and infection control practices.
- Maintain accurate, confidential documentation in the electronic health record and participate in shift handoffs.
Requirements
- Active Licensed Practical Nurse license.
- Local candidates only.
- Ability to work 12-hour shifts in a long-term care setting.
- Strong communication and teamwork skills to coordinate with the care team.
