Responsibilities
- Provide skilled physical therapy services under supervision per the plan of care for residents requiring rehabilitation.
- Assess patient status, document progress, and adjust treatment plans in collaboration with the supervising PT.
- Implement therapeutic exercises, gait training, balance activities, strengthening, and modalities as directed.
- Maintain accurate, timely documentation in the patient chart and Rehab Optima/NetHealth systems as applicable.
- Ensure patient safety, perform falls prevention measures, infection control, and proper equipment management.
- Communicate with the care team and participate in discharge planning and transitional care as appropriate.
Requirements
- Active California Physical Therapy Assistant license
- SNF experience preferred; knowledge of NetHealth (Rehab Optima) preferred
- Ability to work the listed shift (Days) and commit to 38 hours per week
- Demonstrated ability to function effectively in a fast-paced skilled nursing setting
- Eligibility to practice as a PTA in the United States and comply with licensure requirements
