Responsibilities

Requirements
- Active Physical Therapist license in Pennsylvania
- 2+ years of home health experience
- Start of care and revisit experience required
- Ability to work effectively in a home healthcare setting
- Willingness to travel within the defined ZIP code service areas and manage a home-based caseload
